Muddy Creek Charter School was established in 2008 to serve rural communities south of Corvallis. Its use of place-based education integrates the connection to our surroundings, community partnerships, and hands-on project learning activities to serve kindergarten through 5th-grade students from Corvallis and beyond.

TAKE HOME COVID-19 TESTING
Muddy Creek has enrolled in Oregon Health Authority’s COVID-19 Screening Program for K-12 students. This weekly testing program is entirely optional.
To enroll your child(ren) in this program, please fill out the consent form. You can pick up a barcoded test kit on Monday mornings during drop-off, beginning October 11th, and bring it back during drop-off on Tuesday mornings before 10am. The barcode on a kit is specific to Muddy Creek, but not to your child until you link it via LabDash. In order to submit the test, please create a patient account by visiting www.LabDash.net. If you already have an account, you do not need to create another account. LabDash will send you results in 1-2 days.
If your child tests positive, please inform Muddy Creek Charter School immediately.
Instructions on how to test are enclosed with the test kit. Please carefully follow these instructions (being sure to label the test tube with the child’s name or initials AND date of birth). Feel free to contact the lab at (541) 368-3050 or by email at covid@wvtlab.com if you need any assistance.
You should collect the sample on Monday evening or Tuesday morning before school. If you miss this cutoff of 10am on Tuesdays, you may deliver your kit directly to Willamette Valley Toxicology (WVT) at 545 SW 2nd Street, Suite 201, Corvallis, M – F, between 9am and 5pm. Please do not attempt to drop off test kits at the lab’s very busy testing site on 4th St. in the old Baskin Robbins as staff are busy with scheduled appointments. Once the sample has been collected, it should be tested within 72 hours. If you forget to test your child, keep the kit to use the next week.
Sharing of information about your child will only be done for public health purposes and in accordance with applicable law and policies protecting student privacy and the security of your child’s data.
